本人知曉CAACH所進行的各項活動， 作為此處所述活動的參與者， 本人承諾， 對於參與CAACH任何活動中 產生的所有風險和危險， 或在參與CAACH組織的任何娛樂活動以及在任何娛樂設施的使用過程中發生的任 何疾病或傷害， 本人自願承擔一切由此產生的賠償與傷害， 並承諾不起訴CAACH的負責人 董事， 活動承包人， 志願工和所有員工。
Having been informed of the activities conducted by CAACH, I, as an individual of the participant named herein, release CAACH from responsibility of all risks and hazards incidental to the activities, and agree to indemnify, hold harmless and promise not to sue CAACH, officers, directors, independent contractors, volunteers and all employees of CAACH for any illness or injuries occurring during my participation in any activities and/or use of any recreation facility organized by CAACH.
